Collision Between Tanker and Containership – Investigation Report

This incident investigation report refers to the collision between the tanker the British Cygnet and the Containership Vera. On the morning of the 2nd of December the oil tanker British Cygnet was proceeding in ballast on passage from Rotterdam to Fredericia in Denmark to load crude oil. The containership Vera on the morning of the 2nd of December the was on passage from Arhus to Bremerhaven engaged on a container feeder service. At 1130 UTC both vessels entered the buoyed channel north of Fynshoved. The British Cygnet was southbound and the Vera northbound. At 1138 UTC both vessels collided in the channel. Fortunately there were no injuries and no pollution as a result of this incident.

Monthly PSC Report for July 2013

The statistics that are being published each month in the Officer of the Watch blog are based in data available to the Black Sea MoU, the Mediterranean MoU, the Indian Ocean MoU, the Paris MoU and Tokyo MoU. Each month’s PSC report contains also information from the USCG, the MCA, the AMSA and Canadian Coast Guard, these information are presented as reference to the type of deficiencies that are being identified through PSC inspections. Fatal Fall Through “V” Door Onboard Offshore Platform – Investigation Report

This accident investigation report refers to the occupational accident that occurred onboard the platform workover rig Pride 14 on 17 June 2002. The rig was being erected preparatory to conducting workover operations. While the derrick was being raised, a gate across the V-door inadvertently opened. The driller, who was apparently braced against that gate, fell backwards, lost his hard hat, and was fatally injured when he impacted equipment on the lower level, 19 feet below the rig floor.

Death of Second Mate while Evacuating Injured Seaman – Investigation Report

This investigation report refers to the serious injury of an ordinary seaman and the subsequent death of the second mate during evacuation procedures of the ordinary seaman in the port of Alger in 30 September 2011.

Attempts for Pilot Disembarkation in Rough Sea Cause Bulk Carrier Grounding – Investigation Report

This investigation report refers to the grounding pf MV TALI off the coast of Norway on 29 January 2008. In an attempt to disembark the pilot in rough sea resulted in the rudder and the propeller of the vessel being damaged, moreover the propeller shaft moved outwards in such a way that the vessel started making water through the shaft tube. The engine room was filled with water to a height of ten metres, and the vessel lost its manoeuvrability.

Fire on Offshore Platform Caused by HVAC Unit – Investigation Report

This investigation report refers to the fire that broke out on Songa Dee on 4 December 2007. At 13.52 hours on 4 December 2007, the fire alarm sounded on the Songa Dee. The living quarters and important rooms such as the radio room, control room and emergency preparedness center were efficiently evacuated during the course of a few minutes. The fire was located in HVAC Unit No.2 and large parts of the living quarters were filled with thick smoke from the cooling and filter unit in the HVAC system. No one was injured during the incident, but the rapid, intense spread of smoke means that the incident is classified as a potential major accident.

Hydrocarbon Leak from Well Flexible Riser – Investigation Report

This investigation report refers to a hydrocarbon leak which occurred from a well flexible riser during a planned shutdown on Visund on 9 April 2011. Oil film and gas bubbles were observed in the sea near the production risers on the east side of the facility.
