Seafarer Found Dead in Galley

A Filipino seafarer was found dead onboard the Malta flagged MV Elbella at Marsaxlokk Freeport in Malta last Sunday night (17th of May). The seafarer was found in a pool of blood with several stab wounds and two kitchen knives near his corpse.

Pumpman Dies After Entering Enclosed Space – Investigation Report

This investigation report refers to the death of a pumpman after entering a cargo tank of a chemical tanker without taking any safety precautions. The pumpman entered the cargo tank in an effort to retrieve the sampling device used to to document that the cargo had not deteriorated during transport. The crew managed to retrieve the pumpman from the cargo tank but despite efforts to save his life, he was later declared dead.

MV Selendang Ayu Grounding – Investigation Report

On December 8, 2004 the M/V Selendang Ayu, a Malaysian bulk carrier, ran aground off the coast of Unalaska Island in western Alaska’s Aleutian Islands. The grounding caused the ship to break in half  and resulted in an oil spill of approximately 336,000 gallons of fuel oil and diesel fuel that led to an environmental cleanup lasting until June 2006. During the rescue operations a coast guard helicopter crashed and six of the vessel’s crew died just moments after being rescued, while the last persons onboard the ship, the master and a rescuer, watched a few minutes later the ship breaking in half.

Storm in the Black Sea Sunk MV Volgo Balt 199 & Beached MV BBC Adriatic

The St Kitts and Nevis flagged vessel MV Volgo Balt 199, transporting coal from Russia to Turkey, sent a distress signal near Sile yesterday at 07:30 GMT before contact was lost. The vessel had a crew of 11 Ukrainians and a Russian on board. Turkey authorities have sent immediately to the scene a helicopter and salvage boats. Vessels in the area were asked to assist in Search and Rescue. Furthermore, another freighter located in the greater area of Bosporus, the  Antigua and Barbuda-flagged MV BBC Adriatic with 14 crew on board, is being tossed about in heavy seas after suffering mechanical failure.

Superintendent Dies Onboard Sage Sagittarius

A Japanese superintendent who was put onto a Newcastle coal ship after two suspicious deaths, has become the third person to die on the ship in the last six weeks. Kosaku Monji, 37, was on the Sage Sagittarius on October 6 when it was unloading a cargo of Newcastle coal in the Japanese port of Kudamatsu.

Mysterious Cadet’s Death

The ITF (International Transport Workers’ Federation) is calling for the Panamanian and Mexican authorities to reveal the results of their investigations into the death of the young engineering cadet Dayra Wood Pino (age 22) aboard the Panamanian flagged products tanker El Valencia – and to answer questions about:

  1. her death
  2. why her family weren’t informed about it for several days, and
  3. why the ship was allowed to proceed for an incredible 17 days with her body onboard

Second Death Onboard Sage Sagittarius

Almost a week ago a crewman onboard bulk carrier MV Sage Sagittarius was reported missing, possibly after a dispute about conditions on the ship. An investigation had been initiated by the Australian Federal Police. Now the police is investigating again a second suspicious death on the coal ship Sage Sagittarius. The police has set up a crime scene at the ship, where it is berthed at the Dyke wharves at Carrington in Newcastle.
