Barge Collision With Oil Spill in Houston Ship Channel

Last Saturday at approximately 12:35 p.m., watchstanders at Coast Guard Sector Houston/Galveston received a call from the captain of the 585-foot bulk carrier Summer Wind, reporting a collision between the Summer Wind and a barge, containing 924,000 gallons of fuel oil, towed by the motor vessel Miss Susan.

Parana River Main Channel Blocked by Grounded Bulk Carrier

Port of Rosario, Argentina’s largest port for exporting grain might remain blocked for almost a week due to the grounding of a Bulk Carrier in the main channel of Parana River. The Cyprus flagged MV Paraskevi remains grounded there since last Tuesday 11 March.

Incident Information on Loss of Main Electrical Power at Sea

This incident information refers to loss of main electrical power onboard a bulk carrier and while at sea. The vessel had completed heavy weather preparations on deck, when she approached a forecasted storm with force 8-9 and waves up to 7 metres. Shortly after running into the storm the fire alarm sounded, a black out occurred and the main engine stopped.

Corrosion / Pitting on Ship’s Side

This incident information refer to to heavy corrosion/pitting and subsequent cracking that was discovered in the side shell plating and frames of an old bulk carrier at sea, in ballast condition. The damage was found in way of the pipes connecting the hopper tanks and the top side tanks in cargo hold no. 4.

Death of Chief Engineer While Boarding Bulk Carrier – Investigation Report

This investigation report refers to the death of a relieving engineer while boarding the Hong Kong registered bulk carrier Apollo. The relieving Chief Engineer (C/E), who just arrived at the ship’s side after a journey of about 14 hours from Sri Lanka, fell into the water when he was climbing the pilot ladder to board the vessel.

Incident Information on Grooving Corrosion on Ship’s Side

During a loaded voyage of a bulk carrier it was discovered that water had entered into the double bottom tank No.4, amidships. After inspection it was found that the water was coming through the ship’s side via the access trunk from the top wing tank and then to the double bottom ballast tank. The ingress water was estimated to be approximately 300 cubic meters per hour. Fortunately, the leak was limited to the double bottom ballast tank and the ballast pumps were able to keep the tank empty until the crack could be dealt with.

Corrosion in Double Bottom in Engine Room

Water ingress to the engine room of a bulk carrier was discovered during a voyage. The leakage was found to come through the short sounding pipe of the bilge water holding tank in the double bottom. After the visual inspection of the tank, the sea water ingress was found to come through an approximately 45 mm diameter area of serious pitting/hole in bottom plate (see figure below).

Collision Between Bulk Carrier and Containership – Investigation Report

This accident investigation report refer to the collision of a Bulk Carrier and a Containership. More specifically, on 5 June 2012 at 22:34:40 hours, the Hong Kong flagged bulk carrier SPRING GLORY and the Danish flagged container ship JOSEPHINE MAERSK collided in the eastern approaches to the Singapore Strait, approx. 7 nm NE of Horsburgh Lighthouse. The collision caused damage to both ships and to the cargo of JOSEPHINE MAERSK. In JOSEPHINE MAERSK a fire in the electrical installations of some refrigerated containers was caused by the collision. No one was injured and there was no pollution of the environment.
