Collision Between Bulk Carrier and Containership – Investigation Report

This accident investigation report refer to the collision of a Bulk Carrier and a Containership. More specifically, on 5 June 2012 at 22:34:40 hours, the Hong Kong flagged bulk carrier SPRING GLORY and the Danish flagged container ship JOSEPHINE MAERSK collided in the eastern approaches to the Singapore Strait, approx. 7 nm NE of Horsburgh Lighthouse. The collision caused damage to both ships and to the cargo of JOSEPHINE MAERSK. In JOSEPHINE MAERSK a fire in the electrical installations of some refrigerated containers was caused by the collision. No one was injured and there was no pollution of the environment.

Collision Between Tanker and Containership – Investigation Report

This incident investigation report refers to the collision between the tanker the British Cygnet and the Containership Vera. On the morning of the 2nd of December the oil tanker British Cygnet was proceeding in ballast on passage from Rotterdam to Fredericia in Denmark to load crude oil. The containership Vera on the morning of the 2nd of December the was on passage from Arhus to Bremerhaven engaged on a container feeder service. At 1130 UTC both vessels entered the buoyed channel north of Fynshoved. The British Cygnet was southbound and the Vera northbound. At 1138 UTC both vessels collided in the channel. Fortunately there were no injuries and no pollution as a result of this incident.

Fatal Bulk Carrier and Cargo Ship Collision – Investigation Report

At about 2251 local time on 17 March 2007, a Hong Kong general cargo ship collided with a Chinese registered bulk carrier. At the time of the accident, the weather conditions were fine with northerly wind at force 4. The sea was moderate and the visibility was about 7 nautical miles (n.m.). The Bulk Carrier sustained damages to the bow and the Cargo Ship sank shortly after the collision. Twelve crew members of the Cargo Ship were rescued. Eight crew members were found missing and nine crew members were drowned.

Bulk Carrier and Tanker Fatal Collision – Investigation Report

On the 18th August 2009 at 20:52LMT the Isle of Man registered bulk carrier “Ostende Max” and the Liberian registered oil tanker “Formosaproduct Brick” collided. The collision occurred in the Precautionary Area adjoining the Traffic Separation Schemes in the Malacca Straits to the south west of Port Dickson, Malaysia within Malaysian territorial waters. This case was extremely unfortunate involving a tragic loss of life. Nine crew members were killed with other crew members injured on the Formosaproduct Brick. Three crew members were injured on board the Ostende Max. Both vessels incurred significant fire and structural damage as a result of the collision.

Collisions and How to Avoid Them

The North P&I Club has published during the previous month a new loss prevention guide for watchkeepers on how to avoid collisions at sea. Designed specifically for use on ship’s bridges, it focuses on what the Club considers to be the most important ‘rules of the road’ in the International Regulations for Preventing Collisions at Sea 1972 (COLREGS).
