At about 0510 (UTC) on 16 March 2011, Oliva, a Maltese registered bulk carrier ran aground on the north-west coast of Nightingale Island in the Tristan Da Cunha Group. Oliva was on a loaded passage from Santos, Brazil to China. The vessel sustained severe bottom damage to almost all of her water ballast tanks that resulted in the vessel developing a 12º list to port.

Bulk Carrier and Tanker Fatal Collision – Investigation Report
On the 18th August 2009 at 20:52LMT the Isle of Man registered bulk carrier “Ostende Max” and the Liberian registered oil tanker “Formosaproduct Brick” collided. The collision occurred in the Precautionary Area adjoining the Traffic Separation Schemes in the Malacca Straits to the south west of Port Dickson, Malaysia within Malaysian territorial waters. This case was extremely unfortunate involving a tragic loss of life. Nine crew members were killed with other crew members injured on the Formosaproduct Brick. Three crew members were injured on board the Ostende Max. Both vessels incurred significant fire and structural damage as a result of the collision.
Tanker Grounding due to Uncharted Shoal – Investigation Report
On 11 December 2004, at about 1405, the UK registered tanker British Enterprise grounded in the Port of Istanbul, Ahirkapi Anchorage Area. The vessel was aground for 5 days before she was floated off following a lightening operation. There was no damage to the vessel and no pollution.

On 12 Feb 2009 at 18:30 OOCL Nevskiy berthed at Mussalo Harbour, Port of Kotka. The unloading of its containers began right away. After the incoming containers destined for Kotka
were unloaded, the loading of outgoing containers began at 21:30. The ship’s deck department secured the loaded containers with lashing wires. A seaman working under the supervision of the boatswain was lashing a container on the port side of cargo hatch number 2 when he slipped and fell over the rail. The sea was covered with ice and he was almost instantly killed.

On 17 October 2006, the Hong Kong registered gas carrier Ennerdale experienced a major leak of liquefied propane, while cargo sampling operations were taking place, alongside Fawley Marine Terminal (FMT). The leak was sealed 29 hours later, after an estimated 66 tonnes of propane had been lost to atmosphere.
Electrocution of Fitter During Welding Repair – Investigation Report
On January 2010, the Hong Kong registered general cargo vessel Suntec departed the port of Huangpu, China after loading 19 300 tonnes of river sand to be discharged at the port of Keelung, Taiwan. On the next morning the deck fitter was assigned to carry out electric arc welding repair on the bulwark on the main deck at the port side near a cargo hold. While the fitter was carrying out the welding repair, seawater suddenly rushed up the deck through the opening between the deck and the bulwark plate and dampened his body. He screamed after suffering from electric shock and fell unconscious on the deck.
RoRo Cargo Ship Collision with Moored Bulk Carrier – Investigation Report
On 27 February 2010 the Finnish flagged vessel Global Carrier was on a routine voyage from Turku, Finland bound for Oxelösund, Sweden with a cargo of semitrailers and trucks. There was dense fog in the archipelago but pilot assistance was not considered because the Master was licensed to carry out piloting. The Master had previous experience of this fairway in similar visibility conditions. During a Global Carrier’s maneuver the vessel collided with the moored MV Ecostar G.O.
Cargo Vessel Grounded Minutes Before the Pilot Gets Onboard – Investigation Report
The Dutch-flagged MV Stadiongracht ran aground in the 10.0 metre-deep southern channel to Rauma at 00.15 on 29th December 2010. The grounding occurred in a position which is approximately 2.7 miles (5 kilometres) from the pilot boarding position in the direction to the port. While the pilot was under his way to board the vessel, he noticed the ship’s course and although he managed to contact the Master to change course, the accident was not avoided.
Cargo Ship Collision with Railway Bridge – Investigation Report
The cargo ship RAMONA sailed into the Railway Bridge on 28 March 2012 at 2226 hours. There were no injuries, but very serious material damage to the bridge and minor damage to the ship. It had been agreed between the ship and the bridge keeper that passage could take place at 2226 hours. The bridge keeper’s disposition of the timing up to the passage at 2226 hours was based on specific attention to a train crossing the Railway Bridge and his experience and routine with the typical timing. This led to a very narrow time margin from the train crossing the Railway Bridge to the immediately subsequent opening process for the vessel’s intended passage through the bridge. The ship sailed faster than predicted by the bridge keeper and arrived at the Railway Bridge earlier than the bascule could be opened.
MV Selendang Ayu Grounding – Investigation Report
On December 8, 2004 the M/V Selendang Ayu, a Malaysian bulk carrier, ran aground off the coast of Unalaska Island in western Alaska’s Aleutian Islands. The grounding caused the ship to break in half and resulted in an oil spill of approximately 336,000 gallons of fuel oil and diesel fuel that led to an environmental cleanup lasting until June 2006. During the rescue operations a coast guard helicopter crashed and six of the vessel’s crew died just moments after being rescued, while the last persons onboard the ship, the master and a rescuer, watched a few minutes later the ship breaking in half.
