Monthly PSC Report For November 2012

The following tables summarize some general findings from the PSC inspections conducted in the main PSC MoU areas (Black Sea MoU, Mediterranean MoU, Indian Ocean MoU, Paris MoU and Tokyo MoU) during last month. Data have been also gathered from various Coast Guard agencies and are presented in each month’s report. Data from other PSC MoUs (Vina del Mar, Abuja MoU, Riyadh MoU) could not be extracted at the time this report has been issued and therefore were not included.

The consistency of the information may differ between MoU since there are slight differences on the way each organization presents the inspection info. MV Selendang Ayu Grounding – Investigation Report

On December 8, 2004 the M/V Selendang Ayu, a Malaysian bulk carrier, ran aground off the coast of Unalaska Island in western Alaska’s Aleutian Islands. The grounding caused the ship to break in half and resulted in an oil spill of approximately 336,000 gallons of fuel oil and diesel fuel that led to an environmental cleanup lasting until June 2006. During the rescue operations a coast guard helicopter crashed and six of the vessel’s crew died just moments after being rescued, while the last persons onboard the ship, the master and a rescuer, watched a few minutes later the ship breaking in half.

Seafarer Death After Falling in Frozen Water – Investigation Report

CARISMA had been on a voyage from Slite to Roenne to take a cargo of sand. The ship arrived at Roenne on 25 January 2011 at 2200. As the loading operation was planned to commence the next morning, it was decided not to rig the gangway until visitors were expected. Half an hour after arrival, a part of the crew left the ship to go ashore. Instead of rigging the gang-way they used a pilot ladder that they rigged on the ship side. Shortly after having arrived at the town centre, one of the crew members felt tired and returned to the ship. At 0020 on 26 of January 2012, the seaman was observed struggling in the water between the ship and the quay. Within ten minutes the seaman was safe, but he died later that night at the hospital in Roenne.

Fatal Fall of Shore Worker in Cargo Tank – Investigation Report

At 2258 on 19 February 2010, a German shore worker was fatally injured on board the oil/chemical tanker Bro Arthur. Bro Arthur had part-discharged at Rotterdam before arriving in Hamburg to offload her remaining crude palm oil cargo. A team of three cargo “sweepers” had been arranged under the operational direction of a supercargo (person who has wide experience in cargo operations, and normally acts on behalf of the charterer). While exiting No 2 cargo tank on completion of the “sweeping” operation, one of the “sweepers” fell to the bottom of the tank.

Derailment of On Board Hatch-Lid Gantry Crane – Investigation Report

On 22 July 2011, the hatch-lid gantry crane on board the dry cargo vessel Blue Note derailed while it was carrying a single hatch-lid to its stowed position in preparation for discharging cargo. The derailment caused the chief officer, who had been riding on one of the crane’s wheel units, to be thrown overboard; an able seaman, who had been riding on another wheel unit, to be left hanging by his hands over the 8.4m deep hold; and the second officer, who was operating the crane, to fall to the deck of the control platform. All three crewmen were lucky to escape with only minor injuries.
