This incident refers to a broken rudderstock due to corrosion fatigue. While at sea, the steering failed to respond. Investigations revealed that the rudder was not ίn the position as indicated by the rudder angle indicator on the bridge and in the steering gear room. An underwater inspection was carried out and the rudder was found to have an angle of 90 degrees to port. The rudder was temporarily secured, and the vessel was subsequent!y towed to the port for close-up inspection in dry-dock and for permanent repairs.

This incident refers to the contamination of a ship’s main engine by seawater. The engine room bilges became flooded due to a leaking sea water pipe. Proper actions were not taken when the alarm sounded, so the leakage was first discovered when water had reached the engine room floor plates. Consequently, the leakage was stopped, bilges emptied and the ship resumed its voyage. Shortly after resuming the voyage, the main engine stopped due to low lubrication oil pressure.
Incident Information on Fatal Accident with Lifting Appliance
This incident information refers to a fatal accident involving a lifting appliance. A provision crane loaded pallets onboard a vessel using a pallet fork. After landing the pallet on the vessel’s deck, the pallet fork was cleared and the crane operator moved the crane outward to pick up new load. During this operation, the crane wire broke and the pallet fork and the hook fell down and hit one of the crewmembers on the vessel. Despite the effort of the vessel’s own crew, dedicated first aid personnel and a doctor, the injured person died shortly after the accident.

This incident refers to a ballast tank explosion of an offshore rig during shipyard repairs. During a repair job in a pre-load tank (ballast tank) on board a rig at a repair yard, an explosion occurred in the tank. The rig was undergoing extensive repairs. During the break time, workers using gas torch inside the pre-load tank, had shut the gas torch valves and left the tank. After the break, when they went in again for continuing their job, there was an explosion in the tank as soon as they ignited the gas torch.

Information on Managing Risk While Transporting Fumigated Cargo
This incident information refers to the transportation of fumigated cargoes and how to manage risk resulting from such kind of cargoes. A bulk carrier was carrying a wheat cargo between two ports in Europe. The cargo was fumigated with phosphine gas during the voyage to protect it from insects and rodents. The gas somehow reached the crew’s cabins and a 22-year-old Latvian crew member died from exposure to the gas.

This incident information refers to bearing damage in the main engine of a tanker due to spark erosion. A tanker for oil was on a voyage in the Mediterranean Sea in loaded condition. The crew noted and monitored an abnormally high temperature in one of the main engine’s crankshaft bearings. Suddenly the lubrication oil pressure sank, the oil mist escalated in the crankcase and the engine had to be shut down immediately. The ship experienced a loss of propulsion and drifted for several hours. Finally, the cargo had to be unloaded from the vessel, which was then towed to a repair yard.

This incident refers to cracks identified in a vessel’s propeller hub. When carrying out hull renewal survey in drydock, two open cracks were found during the visual inspection of the propeller hub. The propeller was a 4 bladed mono-block fixed pitch manganese aluminium bronze propeller fitted with cone and key and with diameter: 5205 mm.
