Chemical Tanker & General Cargo Ship Collision – Investigation Report

This investigation report is referring to the collision between the Norwegian chemical tanker Sundstraum and the Malta registered general cargo ship Kapitan Lus. At 1316 LT on 3 July 2009, the Norwegian vessel Sundstraum and the Malta registered vessel Kapitan Lus collided in the Drogden Channel off Copenhagen. Sundstraum was on route from Tjelbergodden in Norway to Stettin in Poland with a cargo of 3707 tonnes of methanol. The vessel was southbound in the Drogden Channel, with the officer of the watch manning the bridge. Kapitan Lus was on its way from St Petersburg in Russia to Le Havre in France with a cargo of 4193 tonnes of aluminium and 182 tonnes of uranium oxide. The vessel was northbound in Drogden and the bridge crew consisted of the Master, the officer of the watch, a Danish pilot and the helmsman. There was good visibility and no wind or seas worth mentioning in the area.

Man Overboard from Chemical Tanker – Investigation Report

This investigation report refers to a man overboard incident that took place onboard a chemical tanker. An ordinary seaman fell overboard while he was trying to undo  a rope lashing that was attached between the bottom of the pilot ladder and a cleat that was recessed in the ship’s hull and used to hold the pilot ladder against the hull. The accident took place during heavy weather and unfortunately the ordinary seaman was not recovered from the sea and must have been presumed dead.

Explosion and Consequent Fire onboard Chemical Tanker – Investigation Report

This investigation report refers to an explosion and a consequent fire in the forecastle of the 2159gt Isle of Man registered tanker Border Heather on 31 October 2004. As a result significant damage was caused to the vessel’s structure and systems but fortunately no-one was injured and there was no pollution.

Collision Between Tankers During STS Operations – Investigation Report

This investigation report refers to the collision of the Greek registered tanker Saetta and the Marshall Islands’ registered tanker Conger when completing a ship to ship (STS) transfer operation off Southwold, Suffolk on 10 August 2009. Saetta’s starboard lifeboat and davit were damaged. Conger was undamaged and there were no injuries and no pollution.

Pumpman Dies After Entering Enclosed Space – Investigation Report

This investigation report refers to the death of a pumpman after entering a cargo tank of a chemical tanker without taking any safety precautions. The pumpman entered the cargo tank in an effort to retrieve the sampling device used to to document that the cargo had not deteriorated during transport. The crew managed to retrieve the pumpman from the cargo tank but despite efforts to save his life, he was later declared dead.

Collision Between General Cargo Ship and Chemical Tanker in Dover Strait TSS – Investigation Report

On 9 October 2001, the 1009 gt general cargo ship Ash was en route from Odense, Denmark, to the Spanish port of Pasajes, with a cargo of steel coils. She had six crew on board and was making a speed of about 6.25 knots over the ground in the south-west traffic lane of the Dover Strait TSS to the south-east of Hastings. The 4,671 gt chemical tanker Dutch Aquamarine was also on passage in the same traffic lane, en route from Antwerp to Swansea. She had a mixed chemical cargo and a crew of 12 on board. She was making about 12.5 knots over the ground. Despite the fact that the weather was fine and the visibility good the watchkeeper on Dutch Aquamarine did not notice Ash in the period immediately before the collision until she was very close and right ahead. Ash was holed in the collision, she listed quickly to starboard, capsized and sank. The six persons on board jumped into the water and five of them were rescued by Dutch Aquamarine’s fast rescue craft.

Explosion During Tank Cleaning Operation

A vessel was in ballast condition and undertaking tank cleaning prior to entering loading port. The crew was cleaning the No. 2 port cargo tank by spraying toluene from the top of the tank cleaning hatch opening into the tank. During the cleaning operation a flashback explosion occurred, killing one crew member and injuring another. This incident took place on a chemicals tanker in 1996.

Tanker Harbour Feature Allision with Bridge

The 473-foot tanker Harbour Feature was successfully freed at approximately 6 p.m., Monday after alliding with the Sarah Long Bridge, on the Piscataqua River in Portsmouth. The tanker became disabled and allided with the bridge at approximately 1:30 p.m., Monday. At this time, the vessel is carrying an unknown amount of tallow oil.

Chemical Tanker Washed Up by Hurricane Sandy

A 170 foot long vessel was washed aground on Staten Island by Hurricane Sandy. The MT John B. Caddell is a Liquid Oil/Chemical Tanker built in 1941. The tanker was moored about a mile from where she ran aground on Front Street, Staten Island. Fortunately nobody was hurt during the incident.

Tanker Bunga Alpinia Inferno

A 38,000 dwt chemical tanker ‘Bunga Alpinia’ was almost destroyed by fire and explosions yesterday morning, while loading methanol at Labuan. It is believed that the accident was caused by lightning, due to heavy rain during the loading procedures, which resulted in several explosions.
