Seafarer Death After Falling in Frozen Water – Investigation Report

CARISMA had been on a voyage from Slite to Roenne to take a cargo of sand. The ship arrived at Roenne on 25 January 2011 at 2200. As the loading operation was planned to commence the next morning, it was decided not to rig the gangway until visitors were expected. Half an hour after arrival, a part of the crew left the ship to go ashore. Instead of rigging the gang-way they used a pilot ladder that they rigged on the ship side. Shortly after having arrived at the town centre, one of the crew members felt tired and returned to the ship. At 0020 on 26 of January 2012, the seaman was observed struggling in the water between the ship and the quay. Within ten minutes the seaman was safe, but he died later that night at the hospital in Roenne.

Two Crewmembers Overboard Cargo Ship North of Terschelling

The 153 m MV Timberland lost two crew members overboard 110 km north of Terschelling on Nov 25, 2012. at 4.05 p.m. Immediately a distress signal was sent, but two helicopters were struggling to stay aloft. The German Navy deployed one helicopter from Borkum which sighted two lights, but did not have enough fuel to investigate this further.

Drunken Master Causes Allision

The cargo ship MV Warnow Jupiter, being enroute Rotterdam-Wismar, allided with the gate of the new Northern Lock of the Kiel Canal in Brundsbüttel in the night of Nov 21, 2012 short past midnight. The police breathalyzed the captain who was found to be intoxicated with 1,05 per mille.

Derailment of On Board Hatch-Lid Gantry Crane – Investigation Report

On 22 July 2011, the hatch-lid gantry crane on board the dry cargo vessel Blue Note derailed while it was carrying a single hatch-lid to its stowed position in preparation for discharging cargo. The derailment caused the chief officer, who had been riding on one of the crane’s wheel units, to be thrown overboard; an able seaman, who had been riding on another wheel unit, to be left hanging by his hands over the 8.4m deep hold; and the second officer, who was operating the crane, to fall to the deck of the control platform. All three crewmen were lucky to escape with only minor injuries.

Black Elk Energy Offshore Platform Explosion in Gulf of Mexico

Coast Guard crews are searching for two missing crewmembers following a platform explosion approximately 20 miles southeast of Grand Isle, last Friday morning. Eleven crewmembers have been medevaced from the Black Elk Energy-owned platform to various local hospitals. Nine additional crewmembers were safely evacuated off the platform. The fire has been extinguished, and a sheen has been spotted in the area. The Coast Guard is searching approximately a 1,400 square-mile area around the platform.

Crewmen Die After Inhaling Gas Onboard LPG Carrier

Five crewmen died died after inhaling gas leaking from a chamber of the vessel. The vessel was the 35,559-cbm LPG carrier Maharshi Krishnatreya of Varun Shipping. The incident took place in India. Another seafarer was airlifted to hospital in the town, where he was said to be in a serious condition on a ventilator. The vessel was manned by 20 seafarers in total.

Oil Tanker Runs Aground due to Cyclone

MT Pratibha Cauvery, an oil tanker with a 37-member crew on board ran aground off Chenai coast as a result of strong winds and heavy weather due to cyclone Nilam. Operations were launched by local authorities to rescue some crew members because the life boat carrying them capsized. One sailor died and three others are said to be in a serious condition.The local authorities are searching for the remaining 5 crew members.

Cargo Ship Sinks off Sri Lanka

On October 30, the Vietnamese vessel Saigon Queen, with a crew of 22 on board, sank off Sri Lanka. MV Saigon Queen was a 102 m cargo ship loaded with a cargo of timber destined for India. The accident took place under heavy weather and the Vietnam MRCC was not able to establish communication with the vessel under distress.

Chemical Tanker Washed Up by Hurricane Sandy

A 170 foot long vessel was washed aground on Staten Island by Hurricane Sandy. The MT John B. Caddell is a Liquid Oil/Chemical Tanker built in 1941. The tanker was moored about a mile from where she ran aground on Front Street, Staten Island. Fortunately nobody was hurt during the incident.
