When a vessel was at anchorage, a diving boat was tied up aft of the vessel for some work. A pilot ladder was rigged to let people climb down to the diving boat. While a person was climbing down, the ladder gave way and suffered total rope failure at 6 different points within the top 4 to 5 meters. The person fell and landed on the work deck of the diving boat approximately 10 metres down from the ship’s poop deck.
A fire broke out at a shipyard in Jurong at about 9.20pm Tuesday. Search-and-rescue work is under way for three people missing. According to the Singapore Civil Defence Force (SCDF) put out the fire at 4.30am Wednesday. The fire had started on a tug boat and spread to three adjacent tug boats at Tanoto Shipyard at Jalan Samulun.

During late January Pacific Rubiales Energy Corp. confirmed the discovery of light oil at the Kangaroo-1 exploration well drilling on block S-M-1101, in the Santos basin, offshore Brazil. The well was drilled as part of an agreement announced by the Company on September 18, 2012, as a minimum work commitment for both the S-M-1101 and S-M-1165 blocks, where the Company holds a 35% participating interest.
During an operational test on board after a repair/overhaul activity, parts of the air-compressor broke loose. In additions to damage to the compressor, one of the parts from the compressor hit the attending engineer with fatal consequences.
During Early February the Spanish Association of Maritime Industries (ADIMDE) and The Basque Maritime Forum have presented the European Commission with a paper which criticises the Dutch financial leasing system for vessels. According to Shangdong province authorities the containership Guangyangxingang sank off the Chinese coast last Monday, due to bad weather and strong winds. One of the crew members has been rescued by a helicopter.
During early February the Norway Petroleum Safety Authority (PSA) issued an order to Statoil following an investigation of the hydrocarbon leak incident on the Heimdal field. The hydrocarbon leak occurred in connection with testing of two emergency shutdown valves on the HMP1 drilling, production and living quarters platform on the Heimdal field on 26 May 2012.
