This incident investigation report refers to the fatal fall from accommodation stairs of a steward onboard a bulk carrier a few years ago. The Steward of a Hong Kong registered bulk carrier fell from near the top of the stairway outside the aft door of the galley down to the floor of the upper deck outside the dry provision storeroom while he was carrying a pile of bowls about 10 to 12 pieces with both hands and ascending the stairs to the galley. Just before the incident occurred, the vessel experienced a sudden and swift rolling movement which caused the Steward lose his balance and fall.
This accident investigation report refers to the occupational accident that occurred onboard the platform workover rig Pride 14 on 17 June 2002. The rig was being erected preparatory to conducting workover operations. While the derrick was being raised, a gate across the V-door inadvertently opened. The driller, who was apparently braced against that gate, fell backwards, lost his hard hat, and was fatally injured when he impacted equipment on the lower level, 19 feet below the rig floor.
The Turkish registered, ISM accredited bulk carrier Gulser Ana entered Stormont Wharf, Belfast on 16 October 2001. On arrival, she underwent a Port State Control inspection by MCA surveyors. Thirty-seven deficiencies were found and the vessel was detained. On the morning of 17 October 2001, two seamen were tasked with freeing up and greasing the releasing hooks on the starboard lifeboat while it was in the water. These had been found seized the day before, during the inspection.
On the morning of May 19, 2000, a contract employee was using a portable of Accident winch (come-along) to remove a section of removable guardrail for the purpose of accommodating the installation of the mounting beams of a temporary crane. One end of the come-along was attached to the section of guardrail to be removed, while the other end was attached to a section of fixed guardrail located approximately 5 feet above the guardrail to be removed. As tension was applied to the come-along, the upper guardrail failed at its base welds and detached from the deck. As a result of the detachment, the employee fell approximately 60 feet to the Plus 10 deck and sustained fatal injuries.
On 12 Feb 2009 at 18:30 OOCL Nevskiy berthed at Mussalo Harbour, Port of Kotka. The unloading of its containers began right away. After the incoming containers destined for Kotka
were unloaded, the loading of outgoing containers began at 21:30. The ship’s deck department secured the loaded containers with lashing wires. A seaman working under the supervision of the boatswain was lashing a container on the port side of cargo hatch number 2 when he slipped and fell over the rail. The sea was covered with ice and he was almost instantly killed.
CARISMA had been on a voyage from Slite to Roenne to take a cargo of sand. The ship arrived at Roenne on 25 January 2011 at 2200. As the loading operation was planned to commence the next morning, it was decided not to rig the gangway until visitors were expected. Half an hour after arrival, a part of the crew left the ship to go ashore. Instead of rigging the gang-way they used a pilot ladder that they rigged on the ship side. Shortly after having arrived at the town centre, one of the crew members felt tired and returned to the ship. At 0020 on 26 of January 2012, the seaman was observed struggling in the water between the ship and the quay. Within ten minutes the seaman was safe, but he died later that night at the hospital in Roenne.
At 2258 on 19 February 2010, a German shore worker was fatally injured on board the oil/chemical tanker Bro Arthur. Bro Arthur had part-discharged at Rotterdam before arriving in Hamburg to offload her remaining crude palm oil cargo. A team of three cargo “sweepers” had been arranged under the operational direction of a supercargo (person who has wide experience in cargo operations, and normally acts on behalf of the charterer). While exiting No 2 cargo tank on completion of the “sweeping” operation, one of the “sweepers” fell to the bottom of the tank.
On 13 December 2010, an able seaman (AB) fell into the River Clyde from the St Vincent and the Grenadines registered cargo vessel Joanna, while the vessel was alongside in Glasgow, Scotland. He was recovered from the water about 25 minutes later, but could not be resuscitated. The investigation identified that the AB almost certainly fell while climbing up to the port side platform of the straddle lift used to move the vessel’s cargo hatch covers.
On 28 January 2009, the chief officer on board the UK registered container ship Ville de Mars fell almost 8 m when descending into a water ballast tank (the vessel’s forward ballast tank). The vessel was on passage in the Gulf of Oman. He was removed from the tank by the ship’s crew and died while being flown to a hospital ashore in Oman by a Royal Navy helicopter. The chief officer had been due to leave the vessel the following day in Jebel Ali, UAE. No postmortem was conducted.
